SERVICES

Verification of Records is one of the most crucial tasks in the office which is utilized by various verification companies may it be local and international.

This varies into two (2) types:

 

ACADEMIC/ SCHOLASTIC VERIFICATION

This type of verification is the usual verification request processed for international and local employment purposes.  Stated below are the process for Verification.

Mandatory requirements

a)     Educational Verification Request (with company’s official letterhead with official signature

b)    Letter of Authorization/ Consent from the candidate to be verified with his/ her signature.

c)     Clear scanned documents of the candidate. (e.g. Transcript of Records, Diploma, Certificate of Graduation, and the likes)

INTERNATIONAL NURSING BOARD VERIFICATION

This type of verification is one of the requirements for International Nursing Board, this includes CGFNS. NCLEX, WES, etc.

 

Procedure of issuance of any documents:

  1. All financial obligations must be settled before approval of request of any documents.
  2. Fix schedule for release of any documents is 3 to 5 working days upon payment of the request.

STUDENT LINK

As general policy, cross enrollment can only be allowed for graduating students with written consent from the student is regularly enrolled in. The School Registrar, in consultation with the VP for Academic Affairs shall decide on application for cross enrollment subject to academic policies and standards.

Proper processing for Dropping and Withdrawal is done by processing the Withdrawal/ Dropping Form to officially validate the process.

Shifting of course can only be permitted under the following circumstances:

  1. The student shall apply for shifting using the shifting form that can be acquired in the Dean’s Office.
  2. A letter of request for shifting stating the reason with signature of the shiftees for shifting must be attached to the form stated above.
  3. An evaluation for shifting will be conducted by the Office of the Registrar for transfer of data to the course/ department to be shifted to.
Graduation Policies
  • There shall be a graduation committee composed of the Registrar (Chairman), Vice President for Academic Affairs (Co-Chair), Deans, Student Affairs and Service Dean, Basic Education Principals, and the College Secretary.

  • Graduation Rites shall focus on the theme agreed and approved by the committee.

  • No student is allowed to attend graduation practices/ rehearsals unless his/ her name is officially enlisted as candidate for graduation form the Registrar’s Office.

  • Graduation practices/ rehearsals shall be announced promptly with strict checking of candidates’ attendance. The graduation rehearsals shall be supervised by the Graduation Committee.

  • Graduation rites should not be exercised in splurging and display of pomp and pageantry but should be a Spartan affair which exhort civil duties, sense of community, personal responsibilities and institutional core values.
